At CardScan, we understand that every person and company has a unique way of doing business, and your lead qualifying needs may be specific to your business. By designing your own custom forms, you capture the information that's important to you and the needs of your organization. How to create a custom form: First, select and download one of the following pre-built form templates. These templates are in Microsoft Word format.
Note: You must order forms through CardScan directly. Forms ordered through a third-party will not include the software file needed for Lead Qualifier software to read the form. Next, modify the template to suit your needs and save it to your computer. Send the modified template file as an attachment via email to forms@cardscan.com. Please type "custom form" in the subject line. Be sure to include your name, phone number and shipping address in the body of the email so we know how to contact you if there are any questions. After design and processing (usually takes 1-2 days), a CardScan representative will email you a PDF proof of your form. Please check this proof carefully, as CardScan is not responsible for typos or other mistakes that are not caught during the proofing process. Reply to the email with "approved" in the subject line, or send a new Word document with any changes. Call 866.334.4509 if you have questions. When your custom forms are printed, you will receive an email confirmation of shipment that includes a tracking number and an installation file. You will need to run the installation file in order to enable your CardScan Lead Qualifier to recognize the custom form information.
